Presciption Refills
​​Any prescription refill must be called into the office 2 days prior to running out of the medication. To place a renewal request by phone, call 352-610-4408 [Ext.1]. Please have the specific medication information ready as well as your pharmacy information when you contact the office.
Patient Portal

If you are a new patient, you may print out the ' New Patient Form ' provided above. If you have previous physician you would like us to obtain records from, please print the Records Request Form and return to any office staff.
 
Please bring the mentioned forms along with your insurance card(s) and a valid photo ID. Please keep in mind that we are unable to accept a new patient without a valid photo ID presented to us.
 
The HIPPA, Financial and Office policies are for your records. You are not required to print and return these forms to our office.

Medical Records
​​In order to obtain your medical records, please contact the medical records department. Records may take up to a maximum of 30 days to obtain. A records request form that may be downloaded here  must be filled out and presented to the office. A charge for certain records may apply. Please inquire with staff.
Referrals
A referral or a prior authorization required for a service may take up 5 to 7 business days or more. We request you contact us no later than 1 week prior to your specialist appointment in order to give us time to obtain autrhorization. Please keep in mind that each insurance company has different response times. When we submit a request, our office must wait until the insurance company sends back the approval.

Appointments
​​Office visits are scheduled by appointment. We understand that there may be circumstances where you might need to come in for an urgent visit. We ask that you call our office and we will accomodate you as soon as possible.
